The rate for point 1 of FE011 is maintained in line with pay code 4225: TUT HRY UQ PEN EX HOLPAY JAN11
The rate is derived from the table below (taken from page 10 of CL 0064/2022).
The calculation is as follows: (38.58+5.26)/108 x 100 = 40.59 per hour. Holiday pay is then processed every quarter @ 8%.
I have attached a pensions document which shows the relationship between pay codes and pay scales (the rates on the document are out of date but it just shows what each pay scale is aligned to, in case it is of any help)
